IPMG is Indiana's preeminent statewide provider of case management services for the Home and Community-Based Services Medicaid Waiver Program. During its tenure, IPMG set a new standard for high quality, measurable and person-centered case management services, and can proudly offer to you: -Person Centered Planning: IPMG has implemented a Person Centered Planning process, based upon nationally recognized philosophies, that ensures that what is "important to" and "important for" you takes center stage in the creation of your Individual Support Plan. We take seriously our obligation to ensure that you are not only accorded the respect that you deserve, but that you are given the tools to make progress toward the life you would like to create. -Educated, Trained Case Managers: The IPMG Case Manager role is to provide you with the support, education and resources you will need to make decisions about how to achieve the outcomes that you desire in your life. They will facilitate your person centered meetings, answer your questions, and help you to understand the complexities of the waiver system. In addition to all mandated State trainings, our Case Managers also participate in IPMG training to increase their overall level of waiver knowledge, advocacy skills, person centered thinking and professionalism. -Intensive Case Management: Some individuals are faced with specific challenges, such as mental illness, that can make it more difficult for them to adapt to living and working in the community. IPMG employs some Case Managers who specialize in meeting the needs of individuals who are dealing with such challenges. If you so desired, we could assess your needs to see if this type of case management is right for you. -Availability: IPMG employs a 24 hour crisis line that is answered by a person who will involve the appropriate IPMG staff, any time night or day, to help you if you feel you are in a crisis situation. Further, if you just have questions or concerns you would like to express, you can call between 8:00 a.m. through 5:00 p.m., Monday through Friday to talk to a Customer Service Representative.
